This form is for both adult and child Chiropractic New Patients. If there are any questions you would like to discuss before signing the informed consent, please bring the form with you and speak to your Chiropractor before you sign.
ADULT NEW PATIENT FORM
This needs to be completed for every chiropractic new patient over the age of 11
Child New Patient form
This needs to be completed for every chiropractic new patient under the age of 11
Massage New Patient form
To be completed for each Remedial Massage new patient regardless of age
Child massage informed consent
To be completed for any child receiving their first massage at the clinic
